Adolescent Diabetes Needs Assessment Tool was developed by a project team spanning Southport and Ormskirk NHS trust, Keele University, Salford University & Edge Hill University. It offers young people visiting the hospital a special code to complete a questionnaire to help them and qualified healthcare professionals assess their management of diabetes.
